Membership Details
Our member-supported model allows us to overcome the multiple challenges faced by primary care providers in Vermont, mainly lack of time and availability. We want to have more time to share our knowledge, help you coordinate your care and collaborate with trusted medical and allied health providers in the community. Our providers will be present and ready to help you when you need us.
